Important documents such as Transcript of Records and Certifications may be issued as requested, regardless of frequency and number of copies. Only the owner of the records is allowed to request for documents in connection with his/her school records and claims the requested documents. Only the Registrar’s office can issue the needed student’s documents that will be used for board exam, employment, and evaluation.
The following certifications are also available ready for issuance:
Valid identification card shall be required for presentation upon request and claiming of the documents for verification and identification purposes. A representative should provide an authorization letter in requesting and claiming the documents. The representative must present one (1) valid ID and one (1) valid ID of the owner of the records. The client is given an option on how to claim the documents either by Pick-up, Proxy or through Fast Courier. Documents not claimed after sixty (60) days shall be destroyed.
On the other hand, processing of TOR for students who transferred to other school will be processed fifteen (15) days after the receipt of the formal request from the requesting school.
